ADVANCE DIRECTIVES: Living Will and Healthcare Power of Attorney

·         The “Living Will” allows you to state your health care wishes if for some reason you would not be able to communicate them. 

·         Who will be your VOICE if you can’t communicate your healthcare wishes?  The “Healthcare Power of Attorney” lets you appoint someone to represent you if you are not able to speak for yourself about your healthcare. 

Trellis Supportive Care will be hosting and facilitating 2 VIRTUAL Advance Directives workshops for Veterans and their families during August.  Both events will be held on the ZOOM platform.  They will begin at 10:00 am on Wednesday August 12 and Wednesday August 26.  To register, or have your questions answered, email or call me:  dtimmons@trellissupport.org   336 331-1309.  For those who register, we will send a package of information and the documents along with instructions about using ZOOM.  There are limited spaces in the workshop, so please register ASAP.
